Job DescriptionJob Description

As a Healthcare Coordinator at Complete Care Partners, LLC, you will play a crucial role in supporting and assisting patients in managing their overall health and well-being. You will work closely with the care team and physicians to ensure that patients have access to the resources they need to improve their health outcomes. 

Responsibilities:

- Collaborate with the care team and physicians to develop and implement care plans for patients

- Coordinate appointments, referrals, and follow-up care for patients

- Assist patients in navigating the healthcare system and accessing necessary resources

- Monitor patient progress and communicate any changes or concerns to the care team

- Maintain accurate and up-to-date patient records and documentation

- Provide education and support to patients and their families on managing their health conditions

 

Requirements:

- High school diploma or equivalent

- Previous experience in healthcare coordination or a related field is preferred

- Strong communication and interpersonal skills

- Ability to work effectively in a fast-paced environment

- Knowledge of medical terminology and healthcare systems

- Ability to maintain confidentiality and adhere to privacy regulations
