The role requires effective communication with diverse individuals and stakeholders, proficiency in English and Spanish, and knowledge of community resources.


Key Responsibilities:
1. Comprehensive Assessment: Conduct thorough assessments of individuals with behavioral health conditions to identify needs, challenges, and strengths.
2. Care Planning: Develop personalized care plans tailored to the unique requirements of each individual, ensuring they address behavioral health concerns effectively.
3. Coordination and Monitoring: Coordinate care services, track progress, and close gaps in care to ensure the fulfillment of behavioral health tailored plans.
4. Communication: Communicate clearly and professionally, both in writing and verbally, with individuals, families, staff, and community partners, fostering effective relationships.
5. Team Collaboration: Collaborate with Care Manager Extenders, facilities, family members, providers, and other stakeholders as essential team members in the care process.
6. Resource Knowledge: Stay informed about community resources and services, guiding individuals towards appropriate support systems.


Qualifications:
• Bachelor’s degree in health, psychology, sociology, social work, nursing, or a related human services field. Alternatively, licensure as a registered nurse (RN).
• Minimum of two years of documented experience working directly with individuals with behavioral health conditions.
• Proficiency in English and Spanish strongly preferred.
• Ability to maintain professional relationships and work effectively with diverse populations.
• Strong communication skills, both written and verbal.


Skills and Abilities:
• Ability to conduct comprehensive assessments and develop personalized care plans.
• Proficiency in utilizing evidence-based interventions and therapeutic approaches.
• Knowledge of community resources and services related to behavioral health.
• Excellent communication skills, enabling effective interaction with individuals and stakeholders.
• Strong organizational skills to coordinate and monitor integrated care effectively.
