HealthCare Support is actively seeking a Pediatric Community Resource Specialist to fill an opening with a fortune 30 company in Florida .

Daily Responsibilities for Pediatric Community Resource Specialist:
  • This position will be field based and will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan provider, providing education on benefits/expanded benefits, and provide community resources to address social determinants of health needs
  • 3 days in the field seeing members then 2 days at home completing documentation/writing up care plans
  • needs- based assessments maybe an hour along, depending on members' needs
  • Quarterly onsite visits for the members - some may need to be seen more frequently but that will be decided when it comes up or if the member needs to be moved to a higher needs team
  • Shift: m/f 8-5.
  • Location: Hybrid Role, openings in Alachua, Baker, Bradford, Citrus, Clay, Columbia, Dixie, Duval, Flagler, Gilchrist, Hamilton, Hernando, Lafayette, Lake, Levy, Marion, Nassau, Putnam, St. Johns, Sumter, Suwanee, Union, Volusia
Required Qualifications for Pediatric Community Resource Specialist
  • Education Requirement: Bachelor degree
  • Years of Experience within Industry: 1+ years of pediatric case management experience
  • Technical Skills: Experience connecting families/children to local resources to various services including employment, housing, independent living, justice, foster care, etc
  • Must have a Valid and Active Driver’s license, reliable transportation and insurance
