HealthCare Support is currently seeking a Pediatric Resource Coordination Specialist to join a leading organization in the healthcare sector.


Key Responsibilities:
  • This role involves fieldwork focused on care management, which includes evaluating healthcare needs, connecting families to healthcare providers, educating on available benefits, and offering community resources to tackle social determinants of health.
  • The position requires spending three days in the field engaging with members and two days working remotely to complete necessary documentation and care plans.
  • Assessments based on individual needs may take up to an hour, depending on the specific requirements of the members.
  • Quarterly in-person visits will be conducted for members, with the possibility of more frequent visits based on individual needs or transitions to higher-level care teams.
  • Work Schedule: Monday to Friday, 8 AM to 5 PM.
  • Work Environment: Hybrid model.

Qualifications:
  • Education: Bachelor’s degree required.
  • Experience: Minimum of 1 year in pediatric case management.
  • Skills: Proven ability to connect families and children with local resources across various services, including employment, housing, and healthcare.
  • Must possess a valid driver’s license, reliable transportation, and insurance.
