Schedule: Monday-Friday 8am-5pm

Duties and Responsibilities: 

  • Works to identify appropriate patients in need of COVID 19 vaccine immunizations.

  • Reviews incoming vaccination records for appropriate intervals between doses received, the need to repeat incorrect doses, COVID vaccination history including manufacturer and number of doses received in the series. Once reviewed, these records will be used to determine which vaccines, if any, are needed at the follow-up visits and then the records will be loaded into the patient chart.

  • Coordinates with the Immunization team to create immune prep forms for pediatric patients, immune clinic patients, including COVID vaccination clinics, and others as the need arises.

  • Collaborates with the COVID vaccination clinic to schedule appointments as needed and verify follow-up appointments are addressed appropriately.

  • Assesses, identifies, and assists patients with transportation needs, if applicable.

  • Works with Nurse manager, Director of Clinical Services and CNO to assess the success of the COVID Vaccine administration and identify any barriers to the receiving the COVID 19 vaccine.

  • Collect data and completes required report on utilization and vaccine administration with the State of NH. Report to include monthly first, second and booster doses, Interpreter Service and Transportation utilization.

  • Performs other duties as assigned.


Qualifications: 
 

  • High School Diploma required.

  • Completion of a Medical Assistant Program, or equivalent medical training preferred.

  • Experience with computers.

  • Previous experience in an ambulatory health care environment required.

  • Bilingual in English and Spanish preferred.
