Wellness Assistant-Supportive Living Facility

Wellness Assistant responsibilities:
I. SERVICE DELIVERY
Set up transportation for residents. Give residents a travel confirmation sheet. Follow up on transportation day to ensure resident is ready for transportation and transportation is coming for resident.
Assist Nurse on Duty (NOD) when working with PCPs (Primary Care Physicians) and other medical agencies when they are here to meet with residents. Contact residents to request they come to Nursing area, ensure their timely arrival, and assist medical personnel as requested.
Work on the floor to assist Certified Nursing Assistants (CNAs) when training
Fill in as a CNA (Certified Nursing Assistants) when needed. (CNA responsibilities are detailed below.)
Conduct new hire CNA orientation covering responsibilities, duties, proper procedures, standards, appropriate dress, and behavior as set forth in the employee handbook.
Complete bi-weekly room audits to ensure housekeeping has been done properly and completely.
Work with the DOW and NOD on assigned projects as needed.
II. ADMINISTRATIVE DUTIES
Conduct twice daily audits of CNA Care Stream, ensuring it is complete and accurate. Report any issues or inconsistencies to the DOW or NOD. Monitor the Vitals/Weights in Yardi to ensure resident vitals and weights have been taken as required and that the information is complete. Note any discrepancies and follow up with CNA and DOW. Make corrections and updates as needed.
File needed documents into resident charts.
Maintain and manage Wellness Department supply inventory and records (in/out), restock as needed. Order Wellness supplies. Order supplies as requested for the Nursing Office.
Manage the cleaning schedule for resident assisted devices.
Send quarterlies and face sheets to MCO as requested.
Email resident status change (location) to Management Office
When needed, the Wellness Assistant will assume the role of CNA and execute the job responsibilities as follows:
I. SERVICE DELIVERY
Promote independence of residents while assisting with personal care services: bathing, dressing, toileting, grooming, oral care and other activities of daily living (ADLs) in accordance with Resident Service Plan. Use proper techniques for resident escort and transfer.
Provide medication reminders and document appropriately in Yardi Care Stream.
Assist in serving meals, including clean-up when required.
Prepare food with residents as part of life enrichment activities. Practice safe food handling as trained. Offer snacks and encourage adequate nutrition and hydration for residents.
Provide housekeeping and laundry services in resident apartments as required and according to schedule in Care Stream.
Provide coverage at the Reception Desk as needed. Conduct Daily Resident Checks at resident meals.
II. SAFETY & INFECTION CONROL
Follow proper infection control policies and procedures.
Maintain a clean and safe environment.
III. RESIDENT RECORDS MAINTENANCE
Accurately document resident care information, incidents, and other important resident information. Upload accurate data collection of resident vital statistics, including weight, blood pressure, pulse, respiration, temperature. Keep other members of the team informed of changes/issues.
Implement simple pain scale with residents, upload data.
IV. ILLNESS IDENTIFICATION
Within appropriate timeframe, report to DOW any changes in resident condition: skin, nutrition status, mobility, mental and/or physical function.
Identify signs and symptoms of illness; promptly report information to appropriate personnel.
V. ACTIVITIES PARTICIPATION (as needed during staffing shortages)
Coordinate and implement appropriate daily life enrichment activities according to resident needs.
Participate in resident enrichment and/or group activities with Life Enrichment Director and/or
Skills, Knowledge, Abilities & Qualities Required
Successful completion of Illinois Certified Nursing Assistant course and inclusion on the IL CNA Registry.
Must be able to read, write and follow oral and written direction in the English language at a level adequate to perform the job duties.
Must be able to verbally communicate information to customers and other employees. Must demonstrate effective communication skills with residents, families, staff, and other service providers.
Must be able to consistently document resident care.
Must demonstrate knowledge of personal assistance devices and their applications.
Must demonstrate basic knowledge of medications and effects.
Must demonstrate knowledge and understanding of safety practices, procedures, and maintenance.
Must demonstrate empathy and compassion to connect with customers.
Must be inspired to serve the needs of our customers.
Must use imagination and creativity to motivate self and others.
Ability to effectively interact with the elderly population.
Must maintain and encourage a positive work atmosphere by behaving and communicating in a manner that promotes cooperation and positive, memorable experiences for co-workers, managers, and others with whom contact is made.
Ability to handle several tasks and respond effectively to unexpected changes and requests.
Able to effectively handle stressful situations and work well under pressure.
Must be capable of performing the essential job responsibilities of this position with or without reasonable accommodation.
Experience working with the elderly population is preferred.
