In this role, you will be responsible for coordinating the intake process for new patients, ensuring all necessary information is gathered and documented accurately. As an Intake Coordinator, you will play a crucial role in the smooth and efficient functioning of our home health services. Your responsibilities will include:

  • Receiving and reviewing referrals from healthcare providers, patients, and family members.
  • Gathering all necessary information to initiate the intake process, including welcome calls with patients.
  • Creating and maintaining strong relationships with case managers and referrals
  • Coordinating with the appropriate healthcare professionals to schedule initial assessments.
  • Ensuring all required documentation is accurately completed and submitted in a timely manner.
  • Communicating with patients, families, and insurance companies to provide updates and resolve any issues.
  • Collaborating with the clinical team to ensure a smooth transition of care for new patients.

Requirements

Education: High school diploma or equivalent required. Licensed Vocational nurse, associates degree or higher in health care related field preferred.

Experience: Minimum of 2 years of experience in a healthcare setting, preferably in intake coordination or patient services.

Skills: Strong organizational and multitasking abilities. Excellent communication skills, both written and verbal. Proficiency in electronic medical record systems, Slack, Microsoft Office, Google Suite.

Knowledge: Familiarity with home health services, medical terminology, and customer service

Ability to work independently and collaboratively in a fast-paced team environment.
