Job Summary:

The Community Relations Specialist will build and strengthen community relationships for PACE of the Southern Piedmont (PoSP). This role is responsible for carrying out efforts that shape the community’s opinion of PoSP and increase awareness of its brand. The Community Relations Coordinator will build positive relationships with stakeholders, organizations, vendors, referral sources, and the general community. This role will perform tasks to support on-going recruitment and enrollment of PACE participants into the program and the growth and maintenance of an optimal enrollee census.

This position requires continuous interaction with referral sources, organizations, community members, teammates, participants, caregivers, vendors, and visitors. The position requires compliance with state and federal PACE regulations related to the role. The individual in this position must be knowledgeable and compliant with PoSP’s values of commitment, integrity, respect, compassion, and collaboration.

Key Responsibilities:

  • Coordinate with the Benefits Counselor to complete intake home visits for potential PoSP participants.
  • Evaluation of potential enrollees to determine participant needs and eligibility for enrollment.
  • Scheduling of assessments that are completed in the home and at the PACE Center.
  • Assists with the PACE Medicaid application process and serves as a liaison between the local Departments of Social Services to assure timely completion of PACE Medicaid applications.
  • Participates in the Interdisciplinary Team and other related meetings and activities as designated.
  • Maintain up to date information on process workflows and referral log
  • Confirm program eligibility with external stakeholders
  • Gather medical records for primary care provider review
  • Key FL2 in NC Tracks system for nursing home level of care prior approval
  • Manage lead information in electronic medical record
  • Scan and upload all initial documents into the electronic medical record system
  • Communicate with PACE Teammates regarding planned enrollments
  • Assists with the development, implementation, and evaluation of the overall marketing strategy for the recruitment and retention of PACE participants and maintenance of an optimal enrollee census.
  • Raise awareness of PoSP throughout assigned territory and/or service area.
  • Develop relationships with referral sources and represent PoSP at community events to generate leads for enrollment.
  • Educate community organizations and the general public on PACE services and enrollment eligibility though virtual and in-person presentations and public speaking opportunities.
  • Carries out tasks as outlined within the organization’s strategic plan and goals.
  • Seek opportunities and conduct business development with potential vendors that will enhance accessible care and services to PoSP participants.
  • Coordinate with Supervisor and Public Relations Specialist regarding marketing and promotional materials.
  • Maintains relevant data tracking tools and reports to capture outcomes.

Miscellaneous:

  • Maintains confidentiality of information.
  • Maintains a safe and clean working environment by complying with procedures, rules, and regulations, adhering to infection-control policies and protocols.
  • Complies with all company safety and risk management policies and procedures.
  • Attends required training programs (classroom and virtual), to include computer training.
  • Analyzes situations requiring a response and initiates appropriate action.
  • Works well independently and takes charge of areas of responsibilities.
  • Manages time appropriately.

Education: A Bachelor’s Degree in marketing, public relations, or another related field.

Licensure, Registry, or Certification Required: Valid North Carolina driver’s license and an acceptable motor vehicle record as defined by the Acceptable Motor Vehicle Record (MVR) chart. CPR and First Aid certification required.

Special Training: Meet a standardized set of competencies for the specific position description established by PoSP before working independently.

Experience: Five years of professional-level experience in the marketing of products or services. Two years of this experience must have involved the marketing of healthcare services or insurance products. Experience in business development with proven relationship-building strategies preferred. Must be a motivated self-starter who can work independently. Business etiquette and a professional approach to dealing with the public are necessary.

Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.

Participant Interface: Must be comfortable and eager to engage our participants in a warm and welcoming manner while maintaining a professional distance and attitude. Experience with issues of aging and health challenges. Demonstrates knowledge of and supports PACE mission, vision, values, standards, policies and procedures, operating instructions, confidentiality standards, and the code of ethical behavior.
