The Senior Manager, Model of Care Innovation will be a part of the Clinical Transformation Team. The Clinical Transformation team leads the DaVita Village in the transformation of kidney care by enabling care teams to engage patients in reaching their health goals and reducing the overall cost to the healthcare system. In an integrated care environment, it is critical to provide the comprehensive and preventative care each patient needs to be healthier in the new value-based healthcare world. The Clinical Transformation team sets the integrated care clinical strategy, develops and innovates on the integrated care delivery model, and partners closely with field operators to deliver high-quality care to Chronic Kidney disease (CKD) and End Stage Kidney Disease (ESKD) patients in our Integrated Kidney Care (IKC) programs.

The Clinical Transformation team is led by the Chief Development Officer and is made up of highly motivated individuals with consulting, process engineering, change management, and clinical experience This highly visible and cross-functional team works directly with Kidney Care and Integrated Kidney Care leadership.

Responsibilities include, but are not limited to:

  • Drive the development, pilot, deployment, and outcomes of innovative initiatives to improve the care of patients in our integrated care models
  • Collaborate and coordinate with teams across the enterprise for insight generation and to deploy new initiatives
  • Engage partners to understand different perspectives to arrive at creative solutions for obstacles identified
  • Convey findings to leadership in a cohesive story, including summarizing insights
  • Develop and maintain relationships with field operators and clinicians
  • Creation of project materials such as training presentations and job aids
  • Travel up to 25% as needed

Qualifications:

  • Bachelor's degree required, Master's preferred
  • 3-5 years of strategy, project coordination/management, program management, or operational improvement experience within a consulting or corporate environment
  • Proficiency in PowerPoint, Excel, and other Microsoft products

Preferred Additional Skills:

  • Problem-solving skills: Ability to balance strategy with execution on tough problems. Comfort with ambiguity and navigating resistance
  • Project management skills: Ability to drive multiple lanes of work forward simultaneously while identifying and solving potential risks that may hinder timely project execution. Ideally, experience launching pilots and deploying initiatives more broadly
  • Collaboration skills: Ability to work effectively, build trust, communicate, and partner at all levels of the company - from front-line teammates to senior executives and physicians. Ability to influence direct and indirect stakeholders, lead large meetings, and facilitate conversations to drive alignment and results.
  • Communication skills: Ability to present ideas, project status, and project findings to various levels of leadership. Ability to create high-quality visuals or collateral to support the message. Healthcare interest (but not experience) required
