Provide program management of the Minor Home Repair / Maintenance / Renovation (MHR) for Mill Levy, Senior Care Act (SCA), and Older Americans Act (OAA) programs.


Conduct project assessments to determine scope of work and need for MHR, Chore and Pest Control Services for persons 60 years of age and older in Sedgwick, Harvey, and Butler Counties.

Develop and conduct educational presentations related to hoarding, home safety, and fall and fire prevention for older adults.

SERVICES MANAGEMENT & ASSESSMENTS

  • Assess consumer's home for environmental modifications, safety and/or repairs.
  • Assess the consumer's health/medical needs according to Federal, State and local program guidelines, based on the review of a completed assessment (UAI) or performance of a project assessment (Mill Levy).
  • Coordinate requests for heavy duty cleaning (CHORE) and pest control for eligible consumers.
  • Develop consumer specific care plans to address and improve the safety, sanitation and livability of their home.

PROGRAM COORDINATION

  • Provide budget oversight of funding sources for MHR.
  • Complete and distribute Request for Bids to vendors and; complete home modification/repair paperwork, to include data entry of appropriate case notes and information in the Nterfuse database.
  • Review submissions and approve work authorization through the Request for Bids process.
  • Mediate conflict between vendors and consumers.
  • Submit required documents to Department Finance staff for processing of vendor invoices.
  • Produce documentation and other reports as required by established deadlines.
  • Provides administrative support to Hoarding Coalition and Hoarding Support Group.
  • Manages special projects as needed including the Hoarding Conference, City of Wichita neighborhood cleanups, and other projects as assigned.

COMPLIANCE AND QUALITY STANDARDS

  • Conduct program review to ensure program requirements, reporting of outcomes and statistics, and vendor billings are in compliance with Sedgwick County and Department policies and meet industry standards.
  • Conduct a post work visit to review services rendered to MHR consumers to assess quality and completion of contracted service.
  • Provide program quality assurance through the continual monitoring of vendors to ensure program compliance, quality standards, and consumer satisfaction is met.
  • Conduct quarterly MHR vendor technical assistance meetings to review Sedgwick County vendor rules, program policies, requirements and updates.

Minimum Qualifications:

Bachelor's degree from an accredited college or university in Social Work, Gerontology, Nursing, Counseling or relevant field of study.

One (1) year experience in the field of aging, working with people with disabilities, and/or brain injuries, or equivalent combination of training and experience that provides the required knowledge, skills, and abilities.

Access to reliable, personal transportation.

Per Sedgwick County Policy, this is a driving level position that requires a valid US driver's license without restrictions and current proof of automobile insurance.


Preferred Qualifications:

Master's degree from an accredited college or university in Social Work, Gerontology, Nursing, Counseling, or relevant field of study.

Two (2) years program / project management experience. Two (2) years' experience in the field of aging for an Area Agency on Aging.

Experience working with individuals with physical disabilities (PD), brain injuries (BI), persons with a mental health diagnosis, and/or individuals with intellectual or developmental disabilities (ID/DD).

Knowledge of construction and ADA standards. Bilingual in English and Spanish.
