About the Role:
- Provide high-quality patient care in the comfort of their own homes
- Work collaboratively with a multidisciplinary team to develop and implement patient care plans
- Conduct thorough assessments, document progress, and report changes to physicians
- Educate patients and their families on disease management, medication, and home safety
- Ensure seamless transitions of care and coordinate discharge from services
What We Offer:
- $60,000 - $65,000 per year
- 1:1 nurse-to-patient ratios for meaningful patient outcomes

Requirements:
