Summary

Within the Capacity Management division, the Admission Triage Coordinators, in partnership with Patient Flow Coordinators (PFCs) and Admission Triage Associates (ATAs), coordinate and optimize patient flow across Adult Hospitals, Mott Hospital, and the Adult Emergency Department. In order to promote quality and safe patient care, Admission Triage Coordinators conduct clinical and financial pre-admission screening of all unscheduled outside hospital transfer patients. Admission Triage Coordinators utilize Michigan Medicine clinical standards and financial guidelines in order to maximize throughput and reimbursement potential for outside hospital transfers into Michigan Medicine. ATCs provide accurate and timely data entry into systems, screen and manage incoming phone calls, support communication with referring customers through continuous assessment of patient acuity and capacity.

Duties include but are not limited to:
  • Function as primary coordinator for transfer requests from other facilities.
  • Screen transfers to evaluate patient care requirements and determine if medical necessity criteria for admission are met.
  • Coordinate and facilitate communication between physicians and requesting transfers and appropriate medical staff.
  • Provide ongoing assessment and communicate changes in patients? clinical condition to Patient Flow Coordinators for proper placement.
  • Collaborate with requesting and admitting physicians to assure appropriate level of care is assigned and written order is obtained.
  • Confirm and document admitting service and attending physician.
  • Evaluate all patient payment sourced and determine authorization requests.
  • Offer air ambulance transportation for ICU patients and refer to survival flight.
  • Obtain and convey clinical information to payers in order to obtain authorization for incoming transfer patient admission as well as air transportation authorization when necessary.
  • Suggest alternatives when a Michigan Medicine inpatient transfer is not indicated and assist outside hospital as needed.
  • Facilitate the AOD (Admitting Officer of the Day) program and provide feedback of process as appropriate.
  • Provide service excellence and effective communication with team members, other departments and external customers.
  • Manage multiple competing tasks and deadlines with energy and efficiency.
  • Additional duties and responsibilities as assigned.
What You'll Do

AOD (Admitting Officer of the Day) Program
  • Admission Triage Coordinators rapidly respond to outside hospital requests for inpatient-to-inpatient transfers, by connecting the Referring Provider with an AOD
  • Efficiently and consistently prioritize which patients are accepted to Adult and Mott Hospital when bed resources are scarce, especially during periods of high occupancy.
  • When patients are declined, provide service excellence and support where possible to our Referring Providers and their patients.
  • The ATC will inform the AOD and Referring Provider about Michigan Medicine bed status, gather relevant insurance information and Referring Provider/hospital contact information, and record all the clinical and logistical information into the MiChart Intake Note, including any special bed needs (such as precautions). While participating on the 3-Way Calls, technical/logistical questions will be addressed by the ATC
  • Document key clinical information and capture the rationale for the AOD's decision.
  • The ATCs coordinate with Survival Flight if needed.
  • The ATCs page the admitting service attending (and fellows/residents, as appropriate) to alert the physician to an accepted outside hospital transfer.
  • ATCs will request OSH records relating to the patient.
  • ATCs complete the Accept or Decline note in MiChart, and continue to make notes regarding the status of pending arrivals within that same note.
Leadership
  • Educate and provide feedback to nursing and medical staff in clinical departments, inpatient units and procedural areas to improve patient placement and facilitate timely transfers.
  • Participate in the orientation and training of on-boarding capacity management staff.
  • Participate in activities to promote own continued learning and professional growth.
  • Support the education mission by providing education and precepting to nursing students as appropriate.
Continuous Quality Improvement
  • Participate in the development, implementation and ongoing evaluation of policies and procedures related to transfer guidelines and utilization management.
  • Assess and evaluate new or revised systems which support Capacity Management goals and objectives.
  • Recommend and facilitate improvements in the patient transfer process based on data analysis.
  • Document notable events for follow-up and make recommendations for quality improvement.
  • Maintain required documentation to support quality improvement and performance metrics.

Required Qualifications*
  • Bachelor's degree in nursing or a related allied health discipline or equivalent combination of education and experience is preferred.
  • Minimum of 2 years of clinical experience.
  • Proven ability to prioritize, meet deadlines and produce detailed and accurate work.
  • Demonstrated ability to exercise good judgment and work successfully within a team and also independently.
  • Must be detail oriented and organized with demonstrated ability to prioritize, multi-task and make difficult decisions under pressure.
  • Excellent clinical knowledge and assessment skills to evaluate and prioritize patient transfer issues.
  • Demonstrated ability to establish excellent working relationships with physicians and all levels of staff.
  • Excellent computer skills (especially in Epic and Interaction Client).
  • Proven ability to assess and handle highly sensitive and confidential matters with considerable discretion and independent judgement.
  • Demonstrated positive work ethic and strong sense of teamwork/problem solving with co-workers.
  • Demonstrated team spirit, initiative and interpersonal skills to effectively interact with team members, medical staff, patients and families.
Desired Qualifications*
  • Working knowledge of Utilization Review and/or experience with patient health outcomes and understanding of reimbursement regulations and issues.
  • Experience facilitating patient placement.
  • Experience with systems that support patient flow, revenue and admission processes.
  • Understanding of University policies and procedures.
