MedStar Health is looking for a Diabetes Care Education Specialist to join our team This position is fully remote, however you must have or be eligible to obtain D.C licensure.

As a Diabetes Care/Education Specialist, you will deliver comprehensive and seamless services that bridge the gap between the clinical and self-management aspects of diabetes and cardiometabolic care. You will be an integral part of the health care team that provides collaborative, comprehensive and person-centered care, and education conducive to behavior change and improved quality of life across the lifespan.

Primary Duties:

· Provides quality diabetes self-management education and medical nutrition therapy based on assessed needs in individual and group classes which are in-person or via telehealth.

· Completes comprehensive assessments for each patient including emotional and behavioral health, interprets personal health data, develops an individualized care plan based on the patient's assessed needs and goals and promotes successful self-management.

· Conducts MedStar Diabetes Boot Camp visit one and visit two, follows the patient until s/he is transferred to the Virtual Clinic, uses MedStar Diabetes Medication Algorithm to perform shared decision making and to collaborate with Boot Camp NPs in the development of a diabetes medication plan that is patient centric.

· Collaborates, advocates, and confers with nurse practitioners, primary care providers, endocrinologists, and other diabetes stakeholders in developing patient centered diabetes plan.

Qualifications:

· Bachelor's degree in Nutrition/Dietetics.

· Registered Dietitian license - Registered Valid license based on location of practice.

· 1-2 years related experience as a registered dietitian.

· CDE - Certified Diabetes Educator Certification; if not already DCES or BC-ADM will sit for the certification examination within 24 months of hire date.

· Diabetes self-management education and support experience preferred.

· Certified Diabetes Care and Education Specialist - CDCES Certified Diabetes Care and Education Specialist preferred.
