Job Overview

INTRODUCTION
Under the direction of the Nurse Manager and in alignment with clinical protocols from the healthcare team, the Clinical Nurse, RN plays a vital role in managing and coordinating the intricate healthcare requirements of a varied patient demographic. This position involves acting as an educator, planner, advocate, and guide for patients at Unity Health Care. The Clinical Nurse is tasked with delivering direct nursing services, conducting assessments, ensuring continuity of care, and enhancing patient outcomes through systematic processes and hands-on clinical intervention.

KEY RESPONSIBILITIES

The following are essential functions and responsibilities associated with this role:

  • Administers standard nursing practices including patient intake, triage, immunizations, and emergency interventions.
  • Establishes a therapeutic rapport with patients to foster trust and communication.
  • Evaluates and addresses issues related to chronic or acute health conditions such as cardiovascular diseases, diabetes, and respiratory issues, while considering socio-ecological factors affecting health.
  • Formulates comprehensive treatment strategies that activate and connect patients with necessary care and support systems (e.g., family, social services, treatment programs).
  • Guides patients in adhering to prescribed treatment plans and identifies obstacles to compliance.
  • Enhances patient outcomes through effective care coordination and follow-up.
  • Collaborates with the entire healthcare team (including Providers, Medical Assistants, and social services) to ensure timely patient care and follow-up.
  • Engages with team members regarding patients' social and mental health requirements.
  • Utilizes diverse educational methods during nursing consultations to manage complex chronic conditions; these visits may include monitoring blood pressure, family planning discussions, and ensuring immunization compliance.
  • Oversees quality control for glucometers and other waived testing procedures.
  • Works alongside the Nurse Manager and Medical Assistant III to uphold infection control standards.
  • Provides discharge instructions to patients effectively.
  • Monitors patient engagement with health center services; offers education and addresses barriers to receiving appropriate care.
  • Facilitates care coordination post-hospitalization.
  • Implements and tracks data collection and reporting in compliance with institutional and federal regulations.
  • Participates in professional development and attends necessary staff meetings.
  • Documents patient interactions in medical records according to established policies and standards, ensuring accuracy and timeliness.
  • As permitted by nursing practice standards, assists in supervising and delegating tasks to other clinical staff (LPNs, MAs).
  • Demonstrates initiative in identifying clinical improvement opportunities and discussing action plans with relevant personnel.
  • May conduct home visits for patients unable to attend appointments at the health center.
  • Performs additional duties as assigned.

MINIMUM REQUIREMENTS

Education
Graduation from an accredited nursing program with a minimum of an Associate Degree in Nursing (ADN). A Bachelor of Science in Nursing (BSN) must be obtained within five years of employment.

Licensure
Current active DC Registered Nurse License and CPR Certification.

Experience
Two years of clinical experience if holding an ADN; one year if holding a BSN. Relevant clinical expertise may include emergency care, mental health, women's health, pediatrics, infectious diseases, community health, and chronic disease management.

SUPERVISION
This position reports directly to the Nurse Manager.

WORK ENVIRONMENT
This role involves regular interaction with staff across all organizational levels and may include external organizational relationships.

PHYSICAL DEMANDS

  • Requires some walking, standing, bending, and light lifting.
  • Must be capable of prolonged sitting, standing, and walking, as well as lifting and transporting files.
  • Visual acuity and the ability to differentiate colors are necessary for prolonged computer use.
  • May require long periods of telephone use.
  • The work environment may involve stress due to multiple time-sensitive tasks.
  • Must be available to work flexible hours and make sound decisions independently.
  • Ability to manage challenging situations effectively.
