Seeking a Family Nurse Practitioner who is
passionate about whole-person care and committed to delivering evidence-based
primary care services. As a provider, you will work with a team of healthcare
professionals to deliver practical treatment for chronic and acute care needs,
disease management services, and preventive care.

Responsibilities:

·
Provide primary care to 10-15 patients per day,
focusing on prevention, practical treatment for chronic and acute care needs,
and providing disease and case management services.

·
Develop biopsychosocial patient care plans in
partnership with the patient and your care team, including an onsite Whole
Health Coach, that meet the needs of each unique patient.

·
Collect comprehensive health history, perform
appropriate physical examinations, conduct diagnostic testing, and prescribe
medications with the convenience of onsite lab and pharmacy.

·
Assess patient population needs and drive
participation in annual whole health evaluations, health coaching, and other
health promotion activities.

·
Provide clinical leadership, participate in team
meetings, and support change management.

·
Ensure that all customer contractual needs and
expectations are met in partnership with your care team.

·
Rotate with other providers in the region to
ensure coverage after-hours.

·
Participate in organizational activities and
committees to optimize processes and programs.

·
Achieve target objectives by improving the
health status of the eligible population and meeting established benchmarks for
clinical outcomes, engagement, and satisfaction.
