Your responsibilities will include:
- Upholding the mission, vision, and values of Apree Health
- Delivering evidence-based primary care to 10 to 15 patients daily, emphasizing prevention, practical treatment for chronic and acute conditions, and disease management services
- Collaborating with patients and your care team, including an onsite Whole Health Coach, to create effective biopsychosocial care plans tailored to each patient's unique needs
- Ensuring comprehensive care in accordance with the Apree medical home model, which includes collecting detailed health histories, conducting physical examinations, performing diagnostic tests, and prescribing medications – all facilitated by onsite lab and pharmacy services
- Assessing the health needs of the patient population and collaborating with team members to promote participation in annual health evaluations and wellness activities
- Providing clinical leadership through active participation in team meetings, clear communication, prioritization of key initiatives, process improvement, and supporting change management efforts
- Meeting all contractual obligations and expectations in partnership with your care team
- Participating in organizational activities and committees to enhance Apree's processes and programs
- Achieving target objectives by improving the health status of the eligible population and meeting established benchmarks for clinical outcomes and patient satisfaction
You should possess:
- MD, DO, or NP Board Certification with a background in primary care, family medicine, or internal medicine
- A current, valid, and unrestricted medical or ARNP license along with a valid DEA number
- A minimum of 3-5 years of experience in primary care
- At least 2 years of experience in case management with a focus on health prevention and population health management
- Experience in team-based care
- Familiarity with EMR systems, preferably NextGen
- Knowledge of coding and documentation, including Medicare Advantage and HCC
- Proficiency with Google Workspace
