About the Role

As a healthcare provider at Apree Health, your focus is on cultivating meaningful relationships with patients to address their comprehensive health needs. You will engage with 10 to 15 patients daily for appointments lasting between 30 to 60 minutes, supported by a dedicated care team, including health coaches, to deliver holistic care – care that is often relegated to specialists in the current healthcare landscape. Committed to outcomes-driven medicine and data-informed population health, you are passionate about serving others and revitalizing primary care to its original purpose.

About Apree Health

Apree Health is at the forefront of the essential transition to value-based healthcare across the nation. Our model uniquely enables the delivery of comprehensive health services to a diverse patient demographic from a single care center. The Apree model is specifically designed to help individuals achieve optimal social, psychological, and physical wellness – an outcome that is neither likely nor affordable within the existing sick-care framework. The model is anchored by three fundamental elements: high engagement, managed care, and a transformative organizational culture, which collectively work to reduce the utilization of costly services such as emergency room visits and inpatient/outpatient care, thereby enhancing the overall health of the organization. Apree Health is recognized as a primary care organization with validations for its population health outcomes and cost management.
