In this role, you will be responsible for:
- Upholding the mission, vision, and values of Vera Whole Health
- Providing evidence-based primary care to a daily patient roster of 10 to 15, with a focus on preventive care, managing acute and chronic conditions, and delivering disease and case management services
- Collaborating with patients and the care team, including an onsite Whole Health Coach, to develop personalized biopsychosocial care plans tailored to individual needs
- Ensuring comprehensive care aligned with the Vera medical home model, which includes gathering detailed health histories, conducting necessary physical examinations, performing diagnostic tests, and prescribing medications using onsite laboratory and pharmacy services
- Assessing the healthcare needs of the patient population and working with team members to promote participation in annual whole health assessments, health coaching sessions, and various health promotion initiatives
- Providing clinical leadership by engaging in team meetings, maintaining proactive communication, prioritizing essential tasks, driving process improvements, and actively supporting change initiatives
- Meeting customer contractual requirements and expectations in collaboration with your care team
- Participating in after-hours coverage rotations with other providers in the region
- Engaging in organizational activities and committees to enhance Vera's processes and programs, such as weekly provider meetings, Clinical Advisory Committee sessions, and Quality Improvement Committee meetings
- Achieving established objectives by improving the health status of the eligible population and meeting benchmarks for clinical outcomes, engagement, and satisfaction
To be considered for this position, candidates should possess:
- MD, DO, or NP Board Certification in primary care, family medicine, or internal medicine
- Active, valid, and unrestricted medical or ARNP license, along with a valid DEA number
- A minimum of 3-5 years of experience in primary care
- At least 2 years focused on case management with an emphasis on health prevention and population health management
- Experience in team-based care
- Familiarity with EMR systems, preferably NextGen
- Proficiency in coding and documentation, including Medicare Advantage and HCC
- Competence in using Google Workspace
Your compensation will range from $115,000 to $138,000 annually, with eligibility for bonuses based on national averages, which may vary in premium markets.
