Essential Functions

In this role, you will:
  • Uphold the mission, vision, and values of Apree Health
  • Deliver evidence-based primary care to 10 to 15 patients daily, emphasizing preventive care, practical treatment for chronic and acute conditions, and disease management services
  • Collaborate with patients and your care team, including an onsite Whole Health Coach, to create effective biopsychosocial care plans tailored to each unique patient, respecting their diverse backgrounds
  • Ensure comprehensive care in alignment with the Apree health model, including thorough health history collection, conducting physical examinations, performing diagnostic tests, and prescribing medications – all facilitated by onsite lab and pharmacy services
  • Assess the needs of the patient population and work with team members to encourage participation in annual health evaluations, health coaching, and other wellness initiatives
  • Provide clinical leadership through participation in team meetings, proactive communication, prioritization of key initiatives, process improvement, and active support of change management
  • Ensure that all patient contractual needs and expectations are met in collaboration with your care team
  • Participate in organizational activities and committees to enhance Apree's processes and programs
  • Achieve targeted objectives by improving the health status of the eligible population and meeting established benchmarks for clinical outcomes, engagement, and patient satisfaction

Education & Experience

You should possess:
  • MD, DO, or NP Board Certification with a background in primary care, family medicine, or internal medicine
  • A current, valid, and unrestricted medical or ARNP license and valid DEA number
  • A minimum of 3-5 years of experience in primary care
  • At least 2 years of experience in case management with a focus on health prevention and population health management
  • Experience in team-based care
  • Familiarity with EMR systems, preferably NextGen
  • Experience in coding and documentation, including Medicare Advantage and HCC
  • Proficiency with Google Workspace

Compensation: $115k-$138k annual salary & bonus eligible (national average, premium markets may vary)
