Role Summary:
The Social Services Worker plays a vital role in delivering emotional and psychological support to residents and their families. This position is essential in fostering a nurturing environment that promotes the well-being of our residents.

Key Responsibilities:

  • Deliver direct psychosocial support to residents.
  • Conduct comprehensive assessments during admission, following any changes in condition, and on an annual basis.
  • Develop, review, and update individualized care plans and progress documentation.
  • Facilitate coordination of resident appointments with external services such as dental and optical care.
  • Participate in and document discussions during resident council meetings.
  • Assist families in navigating the emotional challenges associated with skilled nursing care and the resident's health conditions.
  • Collaborate with residents, families, and interdisciplinary team members to plan for successful discharges.
  • Provide training sessions for staff on psychosocial matters and patient rights.
  • Oversee and mentor Social Services Assistants.
  • Complete additional duties as assigned.

Qualifications:

  • Education: Bachelor’s degree in Social Work (BSSW) or Master’s degree in Social Work (MSSW) preferred.
  • Licensure: Must hold necessary state licensure.
  • Experience: Minimum of one year in a long-term care setting.

Skills and Competencies:

  • Ability to collaborate effectively within a team.
  • Strong communication skills with residents, families, and staff at all levels.
  • Commitment to maintaining confidentiality.
  • Knowledge of psychosocial practices relevant to long-term care.
  • Proficient in guiding and motivating team members.
