* Duties:

  • Provide
comprehensive and coordinated care to patients in a hospital or managed care setting.

  • Conduct diagnostic evaluations and assessments to determine the healthcare needs of patients.
  • Develop and implement individualized care plans in
collaboration
with the healthcare team.

  • Coordinate and monitor patient care, ensuring appropriate utilization of resources.
  • Collaborate with physicians, nurses, and other healthcare professionals to ensure quality patient care.
  • Conduct regular patient assessments and reassessments to evaluate progress and adjust care plans as needed.
  • Provide
education
and support to patients and their families regarding their healthcare needs and treatment options.

  • Maintain accurate and
up-to-date
medical records, documenting all patient interactions and interventions.

  • Participate in utilization management activities to optimize patient outcomes and
resource utilization
.

* $2,500
sign-on bonus

* Qualifications:

* - Valid Registered Nurse (RN) license in New Hampshire

  • Bachelor's degree in Nursing (BSN) preferred.
  • Minimum of 3 years of experience in case management or a related field.
  • Strong knowledge of medical terminology, disease processes, and treatment modalities.
  • Excellent
communication
and interpersonal skills, with the ability to effectively collaborate with
multidisciplinary
teams.

  • Proficient in utilizing electronic medical records systems for documentation and care coordination.
Ability to
prioritize tasks, manage time effectively, and work independently with minimal supervision.

  • Knowledge of utilization management principles and practices is highly desirable.
  • Experience in primary care or a hospital setting is preferred.
