About the Role

We are seeking a highly skilled and motivated Hospice Community Liaison to join our team at LilyCare of New Hampshire. As a key member of our business development team, you will play a critical role in building and maintaining professional relationships with healthcare decision-makers, including physicians, case managers, discharge planners, social workers, and other healthcare professionals.

Key Responsibilities
  • Business Development: Identify, pursue, and close new business opportunities by developing formal or contractual relationships with healthcare organizations, payors, and other key stakeholders.
  • Sales and Marketing: Implement a sales plan for the assigned book of business, coordinating with operational and clinical leadership to ensure exceptional customer service and support the mission of LilyCare of New Hampshire.
  • Community Outreach: Collaborate with hospice staff to ensure smooth transitions for referrals into the hospice program, and create and deliver in-service presentations to hospitals and other referral sources, educating them on hospice services, admission criteria, and referral processes.
  • Relationship Building: Actively participate in community events to promote hospice awareness and build relationships with key stakeholders, and demonstrate a thorough understanding of hospice benefits and services.
  • Communication: Possess excellent communication skills, both written and verbal, and be able to generate and follow up on leads independently.
Requirements
  • Minimum of 1 year of experience in hospice or long-term care.
  • Proven track record in sales/marketing with documented success.
  • Ability to work with minimal supervision and be self-motivated.
