We are currently seeking a skilled RN - Case Coordinator

Employment Type: Per Diem

Work Schedule: Day shifts with weekend and holiday rotations; 8:00 AM - 4:30 PM

Location: St. Luke's Hospital - New Bedford, MA

Key Responsibilities

Under the supervision of the Manager of Case Coordination, you will undertake all essential functions of a case manager. You will coordinate care and services for assigned patients throughout their healthcare journey. Collaborating effectively with physicians and multidisciplinary team members, both within and outside the organization, you will enhance patient care through efficient utilization and monitoring of healthcare resources. You will facilitate the achievement of desired outcomes for patients, families, and representatives. This role is integral to the core Case Management team, supporting utilization functions, including physician consultations for appropriate level of care assessments, denial prevention, and compliance with regulatory requirements.

Qualifications
  • A Bachelor's Degree in Nursing or a related healthcare field, or equivalent experience is required.
  • A valid Massachusetts RN license is mandatory.
  • Certification in Case Management is preferred.
  • Five to seven years of relevant experience is required.
  • One year of experience in case management is preferred.
  • Essential skills include creative problem-solving, critical thinking, proficient time management, and organizational planning, all grounded in a solid clinical knowledge base.
  • Ability to multitask and manage interruptions in a demanding work environment is crucial.
  • Knowledge of funding sources, resources, services, clinical standards, and outcomes is vital for this role.
  • Must be fully vaccinated against seasonal influenza and COVID-19 or qualify for exemption for medical or personal reasons upon hiring.
Compensation

USD $71.64 per hour.
