Job Summary:

The Cancer Center & breast care client navigator’s primary focus is to provide seamless integration of cancer care in a multidisciplinary setting.

The navigator is a liaison for patients, families, direct care providers, and ancillary clinics across Hospital, community providers, and the health care system to ensure continuity of care.

The client navigator is based in the Hospital Cancer Center, a multidisciplinary center affiliated with NYU Perlmutter Cancer Center and serving a population of medically underserved patients.


Areas of responsibility:

Serves as point of contact for patients and physicians for incoming/outgoing referrals.

Guides patients transferring to Hospital on how to obtain and submit outside medical records, including pathology slides if available.

Orients new patients to clinic and Bellevue as needed, provides support and concrete services.

Facilitates interactions and communication with health care staff and providers.

Schedules patients for appointments.

Schedules radiology and other appointments.

Coordinates screening and outreach programs.

Conducts patient satisfaction surveys.


Education:

High School or GED
