Job Title: Licensed Practical/Vocational Nurse

Company: WellMed, an Optum Company

Description: As a Licensed Practical/Vocational Nurse, you'll be responsible for delivering direct patient care under the guidance of a Registered Nurse. Your tasks include following the care plan, administering treatments, and collaborating with the team to ensure positive patient outcomes.

Primary Responsibilities:

  • Provides direct patient care as outlined in the state Nurse Practice Act and agency policies
  • Implements the care plan established by the Registered Nurse
  • Ensures accurate and timely documentation in line with the care plan
  • Offers patient and family/caregiver education relevant to the diagnosis and care plan
  • Participates in coordinating home health services, communicating the need for other disciplines when necessary
  • Adheres to infection control standards and safety measures
  • Utilizes equipment and supplies efficiently
  • Engages in personal and professional development
  • Follows the Agency's SOPs for documentation submission

Communication:

  • Prepares clinical notes and informs the primary physician as needed
  • Communicates with physicians regarding patient needs and changes in condition
  • Collaborates with the Clinical Team Manager to schedule services and recommend additional care

Additional Duties:

  • Participates in on-call rotations, including weekends
  • Upholds Patient Rights and Privacy Practices
  • Ensures availability of necessary equipment and services
  • Maintains patient case load and documentation levels daily
  • Adheres to work hours and submission policies
  • Demonstrates flexibility and teamwork
  • Maintains confidentiality and professionalism
  • Performs assigned duties diligently

Required Qualifications:

  • High school diploma or equivalent
  • Graduate of an accredited practical/vocational nursing program
  • Current unrestricted license as a practical or vocational nurse in Texas
  • BLS/CPR certification
  • Minimum 2 years of clinical experience, preferably in home health or medical/surgical settings
  • Access to reliable transportation
  • Ability to lift and transfer patients
  • Must meet immunization and screening requirements

Preferred Qualifications:

  • IV Certification
  • Wound Care experience

Physical and Mental Requirements:

  • Ability to lift and transfer patients
  • Ability to handle heavy objects
  • Must be able to sit and stand for extended periods
  • Fine motor skills for equipment operation
  • Ability to comprehend and follow instructions
  • Logical reasoning skills for various situations
