Private Duty Nurse LVN

Aveanna Healthcare is seeking a compassionate and skilled LVN to provide high-quality care to our patients in the comfort of their own homes.
As a Home Health Nurse - LVN, you will have the opportunity to make a meaningful difference in the lives of our patients and their families. You will work closely with our interdisciplinary team to develop and implement personalized care plans that meet the unique needs of each patient.
Responsibilities:
- Conduct thorough patient assessments and develop care plans in collaboration with the healthcare team
- Administer medications and treatments as prescribed by the physician
- Provide wound care and management, including dressing changes and wound cleaning
- Monitor patient vital signs and report any changes to the healthcare team
- Document patient progress and care provided in our electronic medical record system
Requirements:
- Active LVN license in the state of practice
- Current CPR certification
- 6 months of prior experience in home health or a related field
- Reliable transportation and a valid driver's license
