Job Summary

We are seeking a compassionate and skilled Private Duty Nurse - LVN to join our team. As a Private Duty Nurse, you will provide one-on-one care to our patients in the comfort of their own homes. You will work closely with our patients, their families, and our healthcare team to develop and implement personalized care plans that meet their unique needs.

Responsibilities
  • Implement and manage patient care plans in accordance with physician orders and Aveanna Healthcare policies and procedures.
  • Conduct thorough patient assessments and coordinate care with other healthcare professionals as needed.
  • Administer medications, treatments, and therapies as prescribed by physicians.
  • Document patient care and progress in a timely and accurate manner.
  • Provide education and training to patients and their families on disease management, medication administration, and other health-related topics.
  • Collaborate with the healthcare team to ensure seamless care transitions and optimal patient outcomes.
Requirements
  • Active, unencumbered license as a Licensed Vocational Nurse (LVN) in the state where you will be practicing.
  • Current CPR certification from the American Red Cross or American Heart Association.
  • TB skin test (current within the last 12 months).
  • Reliable transportation.
