Assessment and Enrollment Specialist

Reporting to the Manager of Outreach and Enrollment, the Enrollment Specialist will be the point person to schedule and conduct in-home assessments to educate interested individuals (enrollees) and to preliminarily assess for eligibility for the LIFE Pittsburgh program.

Once assessed, will coordinate and support the enrollee through the eligibility process to include Clinical Eligibility determination through the Area Agency on Aging.

Will assist with the coordination with the Inter-Disciplinary team during further assessments, care planning and ultimately, the final enrollment meeting.

Will maintain positive relationships with external social service entities to greater support enrollees during enrollment process. May at times have to work non-traditional hours depending on the unique need of the enrollee.

May at times assist with the promotion and education of the LIFE Pittsburgh program to potential enrollees in community and outreach events.

This position has flexibility with office/home work, but primarily is done by traveling daily in the community across our service area.

Requirements:

Bachelor's degree in Human Service, Social work or related field required. Must have demonstrated experience working with the geriatric population.
