Optimus Health Care is a Federally Qualified Health Center with sites operating in Bridgeport, Stamford and Stratford, CT. We are the largest provider of primary health care services in Fairfield County, proudly serving all patients regardless of their ability to pay.

We are presently recruiting an Intake and Social Services Manager.

POSITION SUMMARY

The Intake and Social Services Manager's primary responsibility is to conduct biopsychosocial evaluations for patients seeking behavioral health services through Optimus Health Care. In addition, they are responsible for the supervision of the Intake and Crisis Evaluator and the care management team. Through this work they are coordinating with operations to schedule evaluations and working with care managers to assist BH patients in coordinating support services. This person also provides support and guidance to the medical teams and front office staff at Optimus to manage crisis intervention that may be needed for their patients.

JOB QUALIFICATIONS/REQUIREMENTS

EDUCATION: Master's Degree in Social Work

EXPERIENCE: Two to four years related post graduate experience in behavioral health, particularly outpatient, required. Experience in completing assessments in an outpatient setting preferred. Two to four years of supervisory experience required.

LANGUAGE SKILLS: Bilingual (English/Spanish preferred)

LICENSURE / CERTIFICATION: LCSW through State of Connecticut Department of Public Health
