Key Responsibilities- Provide Individual, Micro, and Group Therapy Services: Deliver evidence-based therapy sessions to clients, addressing a range of mental health concerns.
- Conduct Biopsychosocial Assessments: Gather comprehensive information about clients' mental health, medical, and social histories to inform treatment plans.
- Develop Treatment Plans: Collaborate with clients to create personalized treatment plans, setting goals and objectives for their mental health journey.
- Provide Ongoing Orientation and Education: Educate clients about Mindful Care's programs and services, ensuring they understand their treatment options and resources.
- Document Session Notes: Accurately record session notes within 72 hours, maintaining confidentiality and adhering to HIPAA guidelines.
- Provide Group Coverage and Supervision: Offer support and guidance to colleagues as needed, participating in monthly supervision and M&M sessions with the clinical team.
- Participate in Orientation and Training: Contribute to the onboarding process for new employees, sharing your expertise and experience to ensure a smooth transition.
- Comprehensive Case Management: Provide ongoing case management services, discharge planning, continuing care services, education, and referrals as needed.
- Clinical Supervision: Offer clinical supervision to master's level Social Workers as needed, promoting their growth and development.
- Confidentiality and Compliance: Uphold HIPAA, state, and federal confidentiality laws, maintaining the highest standards of confidentiality and security.
- Licensed Clinical Social Worker in New York and Connecticut: Possess a valid LCSW license in both states, ensuring you can provide services to clients in these regions.
- Insurance Eligibility: Be eligible for insurance through CMS, ensuring you can provide services to clients with various insurance plans.
- Excellent Communication and Leadership Skills: Demonstrate exceptional communication, patient care, and leadership skills, inspiring confidence in clients and colleagues alike.
- Organizational and Time Management Skills: Possess the ability to stay organized, multi-task, and time-manage effectively, meeting deadlines and delivering high-quality services.
