Social Worker 2 (SW 2)- Expert

Pay Rate:
$31 / hour

Position Shift : 8a - 4pm (30 min lunch)

Minimum Qualifications
Bachelor of Social Work from an Accredited Social Work Program or equivalent education and experience.
Basic computer skills to include, but not limited to, Microsoft Office Products.

Duties:

Completing thorough admission and annual psychosocial assessments in a timely manner,
Completing thorough social histories in a timely manner,
Function as an integral member of an multidisciplinary treatment team and participate in all treatment planning meetings,
In consultation with the SW Supervisor, provide required treatment and therapy sessions and case management functions.
Obtain/maintain accurate family contact information and initiate phone contact on a regular basis,
In collaboration with treatment team members, county liaisons and other stakeholders as identified, serve as the point person for development and implementation of a discharge plan for individuals identified as ready for discharge,
Perform other related social work duties as assigned per NSH Departmental standards.
Work in a Forensic Setting interacting with forensic patients, their attorney and other legal officials.
