Investigator / Case Worker - Adult Protective Services

Job Summary:

We are seeking a skilled Investigator / Case Worker to join our team at Liberty Healthcare Corporation. As an Investigator / Case Worker, you will play a critical role in ensuring the wellbeing of adults living with disabilities by investigating reports of need and mitigating or eliminating imminent risk of abuse, neglect, exploitation, or abandonment.

Key Responsibilities:

  • Conduct investigations in residences to determine if reports are substantiated
  • Document findings and develop plans to ensure the wellbeing of adults living with disabilities
  • Manage active cases and travel throughout the region to conduct investigations
  • Collaborate with other professionals to ensure the best possible outcomes for adults living with disabilities

Requirements:

  • Bachelor's degree preferred, but relevant experience will be considered as a substitute
  • Relevant experience in behavioral health, mental health, or intellectual and developmental disabilities services
  • Knowledge of Social Assistance Management Systems (SAMS) and Home and Community Service Information Systems (HCSIS) is helpful
  • Spanish speaking skills are a plus

What We Offer:

  • Starting pay of approximately $47,000 per year, paid $22.60 per hour
  • Opportunities to earn additional income by working optional overtime and taking on additional cases
  • Biannual performance bonuses between $750 and $1,500
  • 30 days of paid time off annually
  • Other benefits offered
