Job Description:
ESSENTIAL DUTIES & RESPONSIBILITIESProvide appropriate linkages, referrals, coordination, and follow-up to services and supports, including referrals and linkages to long term services and supports.
Identifies barriers for discharge and a plan to address them and ensure outpatient follow up care.
Obtains collateral information and collaborates with clients' families, support systems, outside providers and involved agencies regarding progress in treatment and continuity of care.
Work closely with community resources and organizations in coordinating client services.Complete and file all documentation as required in a timely manner including completing intakes for new Reach clients and discharges for clients ending Reach participation.
Ensure releases of information and other required documents are completed and filed.Attend all team meetings to promote collaboration of service and treatment providers.
Completes all agency and department required trainings.
KNOWLEDGE, SKILLS & ABILITIESEffectively communicate via written, verbal, in person and virtual methods.
Understanding of clinical services and recovery principals.
Ability to work independently and as part of a multi-disciplinary team.
Quick learner with telehealth options and ability to teach consumers telehealth options.
QUALIFICATIONSEducation LevelBachelor's Degree in human services field required. Years of ExperienceExperience working in an addiction and mental health setting. Certification/LicensureValid Florida driver's license, current auto insurance and a good driving record.
