Job Summary

We are seeking an HIV Nurse Care Coordinator to join our team in Bradenton, FL. As a key member of our team, you will be responsible for coordinating HIV patient medical care activities, including data entry and tracking of HIV services. You will also identify patients for inclusion in our program, perform comprehensive patient assessments, and develop individualized patient care plans.

Key Responsibilities
  • Coordinate day-to-day HIV patient medical care activities, including data entry and tracking of HIV services
  • Identify patients for inclusion in our program using available metrics
  • Perform comprehensive patient assessments upon entry to service and periodically
  • Develop and implement individualized patient care plans with a focus on disease management and patient empowerment
  • Assist patients and families with navigation of the healthcare system
Requirements
  • Clear/Active RN or LPN license in the State of Florida required
  • Minimum of 2 years' experience as an RN or LPN performing similar tasks required
  • Minimum of 1 year experience as a licensed RN or LPN working with Hep C and HIV patients preferred
  • Current BLS Certification through the American Heart Association, American Red Cross, or American Safety and Health Institute required
