Empath Health is seeking a dedicated Care Navigator to enhance our service delivery.
Work Schedule:
Sunday 7:30 AM - 4:00 PM, Monday - Thursday 8:00 AM - 4:30 PM
Key Responsibilities:
- Coordinate service inquiries and ensure timely access to the Empath Health Network of Care.
- Deliver exceptional customer service with compassion and responsiveness to all inquiries.
- Facilitate optimal access to healthcare services through various communication channels, including phone, website, and in-person visits.
- Provide tailored information and education about programs and services to meet individual customer needs.
- Assist in scheduling appointments by effectively triaging inquiries to the appropriate programs.
- Collaborate with patients, families, and community providers to gather and input essential information into electronic medical records (EMR).
- Maintain a comprehensive understanding of community resources and share relevant information with patients and families.
- Ensure compliance with regulatory standards by accurately documenting physician certification information.
- Monitor and document incoming faxes and facilitate their processing into the EMR.
- Conduct daily audits to verify inquiries, scheduled appointments, and patient interactions.
- Promote emotional well-being by providing a supportive atmosphere for individuals seeking assistance.
- Build and maintain strong relationships with community partners and stakeholders.
- Proactively address potential issues and resolve complaints to enhance service quality.
- Act as an ambassador for Empath Health's mission and values.
- Utilize agency resources efficiently while balancing compassionate care with business needs.
Qualifications:
- High school diploma or GED; relevant experience or training is a plus.
- Strong verbal and written communication skills.
- Ability to multitask and think critically in a fast-paced environment.
- Proficient in typing and computer skills, with knowledge of Microsoft Office applications.
- Familiarity with HIPAA regulations and basic medical terminology.
- Understanding of healthcare reimbursement processes.
- Willingness to assist with Care Navigation as needed.
