Empath Health is in search of a dedicated Care Navigator to enhance our team.
Work Schedule:
Sunday 7:30 AM - 4:00 PM, Monday - Thursday 8:00 AM - 4:30 PM
Key Responsibilities:
- Coordinate service inquiries and facilitate prompt access within the Empath Health Network of Care.
- Deliver exceptional customer service by demonstrating compassion, empathy, and responsiveness to all inquiries.
- Ensure optimal access to the Empath Health Care Continuum through various communication channels including phone, website, fax, and in-person visits.
- Provide tailored information and education regarding programs and services to meet individual customer needs.
- Facilitate appointments through effective triage using established Care Navigation protocols.
- Collaborate with patients, families, community providers, and team members to gather and input necessary information into the patient's electronic medical record (EMR) accurately and in real-time.
- Maintain extensive knowledge of community resources outside of hospice care and share relevant information with patients and families.
- Ensure compliance with regulatory standards by accurately documenting physician certification information in the EMR.
- Monitor and document incoming faxes, forwarding them to Health Information for appropriate processing.
- Conduct daily audits to validate inquiries, scheduled appointments, and patient interactions.
- Foster social and emotional well-being by maintaining confidentiality and providing a supportive atmosphere for individuals seeking assistance.
- Build and maintain strong relationships with community partners and stakeholders.
- Proactively address potential issues before they escalate into complaints, viewing feedback as an opportunity for improvement.
- Act as an ambassador for Empath Health's Network of Care, balancing compassionate service with efficient resource management.
- Promote and embody the organization's vision, mission, and values while contributing to a high-functioning team environment.
Qualifications:
- High school diploma or GED; or one to three months of related experience and/or training; or an equivalent combination of education and experience.
- Strong verbal and written communication skills with patients, families, providers, and colleagues.
- Excellent multitasking and critical thinking abilities.
- Proficient typing and computer skills, with knowledge of Microsoft Office products; familiarity with Suncoast Solutions is preferred.
- Understanding of HIPAA compliance regulations and basic medical terminology.
- Basic knowledge of healthcare reimbursement and community resources.
- Willingness to assist with Care Navigation coverage as needed.
