Position Overview:

Empath Health is in search of a dedicated Care Navigator to enhance our service delivery and patient support.


Work Schedule:

Sunday 7:30 AM - 4:00 PM, Monday to Thursday 8:00 AM - 4:30 PM.


Key Responsibilities:
  • Manage service inquiries and ensure timely access to the Empath Health Network of Care.
  • Deliver exceptional customer service with empathy and responsiveness to the needs of all stakeholders.
  • Facilitate optimum access to the Empath Health Care Continuum through various communication channels.
  • Provide tailored information regarding programs and services to meet individual customer needs.
  • Coordinate appointments through effective triage, directing patients to the most suitable programs.
  • Collaborate with patients, families, community providers, and team members to ensure accurate and complete referrals.
  • Maintain extensive knowledge of community resources and share relevant information with patients and families.
  • Ensure compliance with regulatory standards by managing physician certification information.
  • Monitor and document incoming faxes, ensuring proper handling and integration into patient records.
  • Conduct daily audits to validate inquiries, appointments, and patient interactions.
  • Promote social and emotional well-being through compassionate communication and support.
  • Build and maintain strong relationships with community partners.
  • Proactively address potential issues to enhance patient satisfaction and prevent complaints.
  • Act as an ambassador for Empath Health, balancing compassionate care with effective resource management.
  • Contribute to a high-functioning team environment by embodying the organization's vision and values.

Qualifications:

High school diploma or GED; relevant experience or training preferred. Strong verbal and written communication skills are essential, along with excellent multitasking and critical thinking abilities. Proficiency in Microsoft Office and familiarity with healthcare regulations are required.
