Role Overview:
We are currently seeking an HIV Nurse Care Coordinator to join our expanding team.
Key Responsibilities:
As an HIV Nurse Care Coordinator, you will be responsible for:
- Overseeing daily coordination of medical care activities for HIV patients, including data entry and tracking of services.
- Identifying eligible patients for the program using available metrics.
- Conducting thorough patient assessments upon entry and periodically thereafter, focusing on patient and family support systems and needs.
- Creating and implementing personalized care plans that are goal-oriented, emphasizing disease management and patient empowerment.
- Assisting patients and their families in navigating the healthcare system.
Qualifications:
To be considered for this role, you should possess:
- A valid and active RN or LPN license in the State of Florida.
- A minimum of 2 years of experience as an RN or LPN in similar roles.
- At least 1 year of experience working with Hepatitis C and HIV patients is preferred.
- Current BLS Certification from the American Heart Association, American Red Cross, or American Safety and Health Institute.
