MDS Coordinator

We are seeking a highly skilled LPN/RN MDS Coordinator to join our team at Braden River Rehabilitation Center. As a key member of our clinical team, you will be responsible for developing, implementing, and coordinating the Resident Assessment Instrument (RAI) process to ensure compliance with professional practice standards, physician's orders, company policies, and local, state, and federal regulations.

• Develop and implement care plans with long-term and short-term goals for improvement
• Conduct meetings with multidisciplinary treatment teams, caretakers, and families
• Ensure accurate and timely completion of MDS assessments and transmission
• Stay up-to-date with Medicare regulations, including PPDPM, OBRA, CAA's, and Care Plans, as well as Medicaid CMI and Quality Measures

• Active, unrestricted LPN or RN licensure in the state of Florida
• Experience in clinical reimbursement and MDS completion, accuracy, and transmission
• Knowledge of MDS, RAI process, and Medicare regulations
