Contracted medical staff will be working alongside uniformed Customs and Border Protection (CBP) Agents/Officers in secured federal facilities along the southwest border of the United States. These secured federal facilities are staffed and monitored by CBP personnel and/or contracted security staff 24 hours a day, 7 days a week to assure a safe working environment to include the co-located medical unit. Contracted medical staff work together with CBP personnel to provide health care services for persons of all ages, to include infants, children, adults, and pregnant females

Mission

CBPs Office of the Chief Medical Office (OCMO) employs a family medicine model to provide frontline medical care for persons in CBP custody. This model provides a team of medical professionals comprised of physicians, advanced practice providers (e.g., physicians assistants, nurse practitioners) and support staff (e.g., Licensed Practical/Vocational Nurses, Paramedics, EMTs, Certified Medical/Nursing Assistants). These contracted medical teams provide basic assessment and medical treatment for persons in CBP custody to include emergency care as needed. OCMO is committed to ensuring a system of care that is ethical, responsible, and accountable through surveillance and monitoring activities that are in place

Licensure/Certification

  1. Candidates must have a full, current, and unrestricted license or registration as a Doctor of Medicine (M.D.) or as a Doctor of Osteopathy (D.O.) valid in the location in which the work is performed. Licensure must be maintained current, and any required education, trainings, continued learning to maintain licensure must be completed within the required timeline for completion by the licensing authority. A temporary license will not be accepted.
  2. Candidate must not have had any professional licensure or certification withheld, suspended, revoked, or denied, and the candidate must not have surrendered or allowed a professional license or certification to have expired or lapsed because of an investigation or disciplinary action.
  3. Candidate must not have been convicted of, pled guilty to, pled nolo contender to, or received probation before judgement for any crime other than a minor traffic violation, the record of which has not been expunged.
  4. Basic Life Support (BLS) Certification must be from one of the following approved organizations:
Experience

  1. Subsequent to obtaining a Doctor of Medicine or Doctor of Osteopathic Medicine degree, a candidate must have had at least 1 year of supervised experience providing direct service in a clinical setting, i.e., a 1-year internship or the first year of a residency program in a hospital or an institution accredited for such training. For purposes of this standard, graduate training programs include only those internship, residency, and fellowship programs that are approved by accrediting bodies recognized within the United States or Canada.
  2. Residency trained and board certified is required.
Education/Training

  1. Degree: Doctor of Medicine, Doctor of Osteopathic Medicine or equivalent from a school in the United States or Canada. The degree must have been accredited by the Council on Medical Education of the American Medical Association, Association of American Medical Colleges, Liaison Committee on Medical Education, Commission on Osteopathic College Accreditation of the American Osteopathic Association, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
  2. Degree from Foreign Medical School: A Doctor of Medicine or equivalent degree from a foreign medical school must provide education and medical knowledge equivalent to accredited schools in the United States. Evidence of equivalency to accredited schools in the United States is demonstrated by permanent certification by the Educational Commission for Foreign Medical Graduates, a fifth pathway certificate for Americans who completed premedical education in the United States and graduate education in a foreign country, or successful completion of the U.S. Medical Licensing Examination.
Major Duties

  1. Serves as the sector/region medical authority within the scope of services provided by DHS/CBP.
  2. Provides medical direction and necessary guidance/oversight as required by state licensure for appropriate advanced practice provider supervision, oversight, consultation, coordination, and direction for all medical services, to include mental health and pediatric care in the absence of a behavioral health advisor or pediatric advisor.
  3. Clinical supervisory authority over physicians, mid-level providers, all clinical staff, all medical unit functions, laboratory, and pharmacy.
  4. Provides on-call coverage for assigned medical units/sector 24 hours a day/7 days a week, however, call coverage may rotate based upon staffing.
  5. Responsible for developing and coordinating all aspects of healthcare delivery at his/her assigned facilities.
  6. May provide direct care to patients in accordance with current accepted practice, licensure, certification, credentialing and or granted privileges.
  7. Provides age-appropriate screening, treatment, and patient instruction pertinent to the discipline and facility setting for acute and chronic conditions for all ages.
  8. Serves as the laboratory director in sites where CLIA waived testing is performed, providing oversight of appropriate diagnostic tests and services, to include monitoring and meeting CLIA requirements.
  9. Acts as a reviewer for all outside referrals/consults to specialty services, to include hospital or emergency care that exceed services offered at the medical unit. Audits referrals and post-referral follow-up, as needed, as part of the medical quality management (MQM) program.

  1. Expert knowledge of the current standards of practice in clinical medicine.
  2. Ability to serve proficiently as a clinical expert, providing technical assistance and consultation to clinic medical staff.
  3. Ability and proficiency to properly use equipment, materials and supplies in simple diagnostic and treatment procedures.
  4. Ability to provide input for congressional inquiries and to effectively communicate with Federal Agencies and other organizations as indicated.
  5. Mastery of managerial and administrative practices, to include expertise in routine management of ambulatory clinic functioning and responsibilities.
  6. High degree of independence, initiative, clinical judgment, and follow-through on a wide range of sensitive, complex, and clinical issues.
  7. Teaching experience acknowledging a level of expertise and capacity for developing and
  8. accessing clinical educational programs.
  9. Knowledge of issues confronting the healthcare delivery system, including specific problems and concerns of special population groups.
  10. Knowledge of program policies and guidelines, and operating procedures relating to healthcare delivery systems.

  1. Ability to utilize telephonic interpreter/translation services to complete assigned duties if not fluent in a language the patient understands.
  2. Ability to adapt to sudden changes in schedules and flexibility in work requirements.
  3. Ability to communicate proficiently in English (verbal and written) in order to develop positive rapport with patients, co-workers and other stakeholders.
  4. Ability to establish and maintain positive working relationships in a multidisciplinary environment.
  5. Ability to navigate in an electronic work environment including electronic health records, web-based training and communications.
  6. Moderate proficiency in, common Microsoft Office programs, specifically Microsoft Word, Excel, Outlook, and SharePoint.
  7. Knowledge of regulations (HIPAA/Privacy Act) regarding the confidentiality of patient medical records and information as well as Personally Identifiable Information (PII).
  8. Ability to initiate and maintain compliance with DHS and CBP policies and any overriding federal standards/policies, or accreditation standards that may apply.
  9. Ability to perform record keeping functions in accordance with program policies and the system of records management.
  10. Ability to work in a multi-cultural and multi-lingual environment.

  1. Ability to use the electronic medical record and maintain an "active status" for use and complete all required trainings.
  2. Ability to utilize the electronic medical records system for all documentation, to include, but not limited to all patient care, monitoring, medication administration/observation documentation, and input of any paper records as applicable.
  3. Ability to maintain non-involvement in the security aspect of patient care and custody.
  4. Assures a safe and clean working environment and assists in the disposal of biologically hazardous waste on a regular basis.
  5. Ability to oversee laboratory services and has awareness and ability to perform quality control of laboratory reagents and perform CLIA waived testing as needed.
  6. Ability to completes all annual training or additional training as directed.
  7. Ability to travel to each assigned medical unit quarterly and be able to work at an assigned medical unit daily to provide oversight as required.
  8. Ability to assist with patient care in time of staff shortage or surge.
  9. Some telework may be considered based upon CBP need.
  10. Ability to serve at or provide patient care at alternate medical units within assigned sector/region on an as needed basis as required.
Supervisory Controls

  1. Works under the administrative direction of the Deputy Medical Director or in his/her absence the Medical Director.
  2. Works under the clinical supervision and/or direction of the Deputy Medical Director or in his/her absence the Medical Director.
In this position, the employee will serve as a contractor to the federal government and does not have any federal authority or rights

Physical Demands

  1. Able to walk, stand, bend, or sit for extended periods.
  2. Able to reach to retrieve documents or supplies.
  3. Able to carry materials and/or file boxes up to 30 pounds on an infrequent basis or assist patients into/out of a wheelchair.
  4. Must be capable of standing on hard surfaces (cement floors) for long periods of time.
  5. Must perform the duties in a stressful and often austere environment without physical limitation.
Must be able to respond to a medical emergency and perform BLS/CPR as needed
