Full Time/Exempt

Responsible for the planning, organization, direction, evaluation, and safe delivery of high-quality care to clients of all ages.

Essential Duties and Responsibilities:

Receives referrals and assigns qualified personnel accordingly. Organizes daily assignments and on-call schedules. Supervises hospice and palliative nursing personnel, providing direction and ensuring quality services. Manages caseloads, conducts home visits, and supports staff professional development. Participates in recruitment, selection, and staff development activities. Reviews clinical records, ensures complete documentation, and participates in program planning and evaluation. Ensures accurate computer entry of patient/job information and participates in administrative call rotations. Communicates effectively with clients and maintains professionalism. Follows work schedules, annual education requirements, and other related duties as needed.

Minimum Qualifications:

Education: Must have a Nursing degree and relevant clinical experience.

Experience: Knowledge of State Regulations, Medicare, and Medicaid standards.

Certificates, Licenses, Registrations: Must hold a current Connecticut Registered Nurse License.

Other: Reliable transportation required.

Day Shift / 40hrs/wk
