Position Details:

  • Location: Glastonbury, CT
  • Department: Internal Medicine
  • Schedule: Full-time, 40 hours per week, Monday through Friday, with shifts from 7:00 AM to 5:30 PM or 7:30 AM to 6:00 PM. This role may include a rotating schedule of 4 days x 10 hours.

In this role, you will support the care team and its patients by delivering and documenting nursing care in accordance with established standards and practices. You will provide patient care and education, assist in coordinating services from other providers, and act as a liaison between patients, staff, and providers. Accurate and timely documentation of all patient interactions and clinical activities in the electronic health record (EHR) is essential.

Key Responsibilities:

  • Administer and document patient care, including injections and preventive screenings
  • Triage patients following approved protocols and scheduling guidelines
  • Prepare patients for consultations by collecting and recording clinical information
  • Educate patients about their conditions and provide self-management instructions
  • Execute standing orders, such as prescription refills and test orders, in line with practice policies
  • Document all patient interactions and clinical activities accurately in the EHR
  • Conduct outreach to patients needing appointments or services, communicate test results, and follow up on care plans
  • Manage patient phone calls, taking detailed messages and routing them to providers as necessary
  • Monitor task lists to ensure timely responses and follow-ups on critical referrals
  • Complete annual CLIA competencies in coordination with Regional Nurse Managers
  • Support the Clinical Lead and serve as a resource for other clinical staff
  • Mentor new clinical team members
  • Maintain open communication with the care team and Regional Management
  • Stay informed about current coding, OSHA, and CLIA regulations
  • Adhere to patient safety initiatives and infection control regulations
  • Perform additional duties as assigned

Required Qualifications:

  • Active, unrestricted RN licensure in CT
  • Minimum of 1 year of nursing experience
  • Intermediate proficiency in Microsoft Office

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN)
  • Experience with electronic health records (e.g., Epic)
  • Experience in a hospital or medical office setting
  • Familiarity with medical terminology

Compensation for this role ranges from $28.03 to $54.95 per hour, based on various factors including local labor markets, education, and experience.
