Position Overview:

Location: Glastonbury, CTDepartment: Internal MedicineSchedule: Full-time, 40 hours per week, Monday through Friday, with shifts from 7:00 AM to 5:30 PM or 7:30 AM to 6:00 PM. This role is eligible for a rotating schedule of 4 days x 10 hours.

Support the care team and its patients by delivering and documenting nursing care in accordance with established standards and practices. Provide patient care and education, assisting in the coordination of services from other providers and resources. Act as a liaison between patients, staff, and providers. Accurately document all patient interactions and clinical activities in the electronic health record (EHR) in a timely manner.

Key Responsibilities:

Administer and document patient care, including injections and preventive screeningsTriage patients based on approved protocols and in line with scheduling guidelinesParticipate as a member of the care team, preparing patients by collecting and recording clinical information as per protocolEducate patients about their conditions and provide guidance on self-managementExecute standing orders (e.g., authorizing prescription refills and ordering tests) in accordance with practice policiesEnsure accurate and timely documentation of all patient interactions and clinical activities in the EHRConduct outreach to patients needing appointments or services, communicate test results, and follow up post-appointments to ensure adherence to care plansHandle patient phone calls, taking comprehensive messages and routing them to providers as necessary using the EHRMonitor task lists to respond to tasks promptly, follow up on overdue orders/worklists, and track critical referralsIn collaboration with Regional Nurse Managers, complete annual CLIA competenciesSupport the Clinical Lead and serve as a resource for other clinical staffMentor new clinical team membersMaintain open communication with the care team and Regional ManagementStay informed of current coding, OSHA, and CLIA regulations, as well as company policiesAdhere to all patient safety initiatives and infection control regulationsPerform additional duties as assigned

Required Qualifications:

Active, unrestricted RN licensure in CT1+ years of nursing experienceIntermediate proficiency in Microsoft Office

Preferred Qualifications:

BSNExperience with electronic health records (e.g., Epic)Experience in a hospital or medical office settingFamiliarity with medical terminology

Connecticut Residents Only: The hourly range for this role is $28.03 to $54.95 per hour. Compensation is based on various factors, including local labor markets, education, work experience, and certifications. UnitedHealth Group adheres to all applicable minimum wage laws.
