The PHACHW I collects and maintainsdata on referrals and outcomes while adhering to establishedprotocols and protecting the confidentiality of patientinformation. MAJORJOB RESPONSIBILITIES

  • Provides screening for social needsand links individuals to appropriate community resources throughreferral and navigation services.
  • Assistsindividuals to develop an action plan to address identified needswhile encouraging the individual to set goals, identify challenges,and monitor their progress.
  • Educates andinforms individuals about healthcare, social services, communityresources, wellness, and disease prevention.
  • Provides culturally appropriate support and informationfor individuals and their families while establishing a trustingrelationship through a series of structured visits and follow-upphone calls in a variety of settings, including localcommunity-based organizations.
  • Remains up todate regarding local community resources to properly assistindividuals to address identified needs.
  • Enhances care quality by aiding communication andbuilding relationships between individuals and healthcare providersand/or community resource providers.
  • Consultswith administrative supervisor and clinical team to coordinateactivities and address identified healthcare needs of theindividual.
  • Assists with chronic diseaseself-management and medication adherence.
  • Ensures education about primary care and appropriateemergency department use and assists individuals to identifyprimary care options and a medical home.
  • Completes appropriate documentation and record keepingthat follows organizational policies and procedures and state andfederal regulations.
  • Works closely withcommunity-based partners to obtain feedback on the results ofreferral and navigation services.
  • Performs allduties in a manner that protects the confidentiality of patientsand does not solicit or disclose any confidential informationunless it is necessary in the performance of assigned jobduties.
