Summary: 

Responsible for working with the treatment facilities to ensure successful coordination of care between levels of care.

Essential Duties and Responsibilities:

Collaborate with facility multi-disciplinary teams to identify gaps in the client care continuum and develop and facilitate the plans that improve client care and services. Create and maintain reports of data while working closely with administration in problem identification and solution. Provide updates and tracking on client navigation through the care continuum to Admissions and Facility management. Coordinate meetings among stakeholders to ensure clients move effectively through the continuum of care. Other Duties as assigned.

Required Qualifications
Licensure, Education, & Experience

Minimum of a Bachelor’s Degree or higher. 5 years of experience in a drug and alcohol and/or mental health setting, preferred.
