Job Summary: Our patient centric and proactive Medical Assistant by trade (formally referred to as a "Chronic Care Associate") will connect and triage patients, answer a wide range of questions, and perform data entry to help demonstrate patient care second to none The "Chronic Care" niche is a new discipline within our team; therefore, desired candidates are self-starters who enjoy learning from the ground up, are not afraid to take initiative, and are able to perform duties outside their job description. The Medical Assistant/Chronic Care Associate position will have support and interactions with several other Clinic members, so preferred candidates are comfortable communicating across various platforms.

Duties of Medical Assistant (Chronic Care Associate): Perform data entry through Electronic Medical Record system Maintain medical records and patient confidentiality  Explains treatment to patients as needed Answer phone calls in a friendly manner and professional manner Other Administrative duties as needed, such as registering patients and scheduling appointments Ability to multitask and move between responsibilities in fluid manner Works with software to help address patient needs Mass weekly check-ins Triage messages and respond in a timely manner Fulfill patient needs, including but not limited to: Medication refills Prior authorizations Chart and lab reviews Manage interactions between providers to patient Communicating with over 500+ patients via texting software Qualifications  of Medical Assistant (Chronic Care Associate) : Demonstrates eligibility to work for any employer in the . High school diploma, GED, or suitable equivalent required. Associate's or Bachelor's Degrees strongly preferred in the areas related to Healthcare and Biology. Previous healthcare experience preferred Previous experience as a Medical Assistant or CNA desired Ability to provide excellent customer service to show superior patient care. Demonstrates enthusiasm and drive.  Able to work compassionately with patients and coworkers to exhibit patient care second to none Exudes respect and flexibility to impact the workplace in a positive manner. Ability to relate and work effectively with others; works well within a team environment Demonstrates excellent skills in verbal and written English communications for safe and effective patient care, in order to meet documentation standards. Willingness to participate in goal-setting and educational activities for professional advancement. Previous experience and familiarity with EMRs is preferred. Is aware of standards and performs in accordance with them. Able to work in a fast paced environment. Strong attention to detail.
