Description


The City of Anaheim Community Services Department seeks a passionate Community Services Coordinator to perform case management of low-income families for the Community Development Block Grant (CDBG) Program. Based out of the Ponderosa Park Family Resource Center, the Community Services Coordinator will work directly to help families stabilize their home environment while assessing their unique needs to create linkages for service delivery and success plans. While most clients will come to the Family Resource Center to seek assistance, the need of the clients may also involve in-home assessments. The Community Services Coordinator will plan, coordinate, and implement Human Services programs, activities, and services of specific functions as it relates to CDBG eligible clientele including overseeing emergency assistance programs.
 
 Candidates must possess responsible experience in Human Services, Youth Development, Counseling, and/or Social Work. Ideal candidates will possess a Bachelor's degree in behavioral sciences, sociology, social work, family services, human services, counseling, or related field. Case management experience with the CDBG program is highly desired.

This CDBG-funded position is subject to grant availability. 
 This position requires candidates must be bilingual in both verbal and written Spanish. 


Essential Functions

The following functions are typical for this classification. Incumbents may not perform all of the listed functions and/or may be required to perform additional or different functions from those set forth below to address business needs and changing business practices.

 Provide case management services including assessing client needs, coordinate a service delivery plan, provide para-professional counseling, and conduct follow-up checks on success of client/agency linkage, and conduct client progress checks.
 
Conduct in-home assessments of family situation/needs and development of service delivery plan; may include conferences with parents regarding status of child welfare; conduct emergency assistance and provide crisis intervention and follow-up to families.
 
Provide in-depth information and referral services; identify on-going support services/needs of families and assist them in accessing those services; identify short-term and long-term needs of clients and provide responsible referrals to social service agencies for assistance; problem solve obstacles to client progress; instruct clients on how to self-assess and access services.
 
Monitor and report client progress toward independence; establish and prepare case management files; prepare and maintain long term tracking records of clients; prepare and maintain statistical and narrative reports, productivity records, daily staff logs, project plans, and incident reports.
 
Participate in planning meetings and implementation of special events, educational programs and health services; conduct parent education presentations on subjects such as family self sufficiency, parenting skills and strategies, and financial budgeting; provide bilingual services including translating and transcribing English to Spanish and Spanish to English, and completion of public assistance forms.
 
Maintain effective communication and working relationships with patrons, participants, clients, co-workers, City Department representatives, and outside agencies.
 
Recommend and assist in implementing goals and objectives within assigned area.
 
Respond to public inquiries and requests; investigate complaints and recommend corrective action as necessary.
 
Participate in the selection of staff and volunteers; provide or coordinate staff training; work with employees to correct deficiencies; implement counseling and disciplinary procedures.
 
Plan, prioritize, assign, schedule, supervise, and review the work of assigned part-time staff.
 
Evaluate operations and activities of assigned responsibilities; recommend improvements and modifications; recommend programs and services for implementation; prepare various reports on operations and activities.
 
Participate in program budget preparation and administration; prepare cost estimates for budget recommendations; submit justifications for assigned events and programs; monitor and control expenditures; order and maintain supplies and equipment.
 
Maintain activity and progress records; conduct research and prepare reports; attend staff meetings; may attend regional workshops and conferences.
 
Promote safety awareness and ongoing safety inspection of equipment and facilities; monitor follow-up maintenance and repair; instill employee safety practices that prevent injuries to staff and patrons and protect City property from damage.
 
Develop and prepare publicity material including news releases, flyers, pamphlets, brochures, and presentations; perform public relations work within the community and with community groups pertaining to specific programs, events, and special activities.
 
Coordinate fundraising endeavors; identify both in-kind and monetary program needs, campaign goals and solicits need support.
 
Coordinate special events and projects, sometimes in collaboration with multiple outside agencies and/or service providers.
 
Perform related duties and responsibilities as required.

Qualifications

Experience and Training: Must possess responsible experience in Human Services, Youth Development, Counseling, and/or Social Work. Candidates with prior case management experience with CDBG and that possess a Bachelor's degree in behavioral sciences, sociology, social work, family services, human services, counseling, or related field are highly desirable.
 
Knowledge of: City, department, division policies and procedures; application of policies and procedures within assigned area; methods and techniques used in marketing and promotion; modern office procedures; field-related software applications; procedures and techniques of budgeting; business letter writing and basic report preparation; principles and procedures of record keeping and data collection.

Ability to: Identify client needs and match to community resources, as well as practice para-professional counseling, behavior guidance, conflict resolution and mediation techniques; communicate clearly, concisely, and effectively in English and Spanish orally and in writing; maintain effective interpersonal communication skills; plan, organize and prioritize tasks; lead, motivate and guide youth participants; prepare and administer a budget; train staff and volunteers; communicate effectively with participants and their families, school administrators, law enforcement personnel, social service agencies, religious community, supervisors, and members of the public; maintain effective communication in hostile environments; use effective conflict resolution and mediation techniques; develop program procedures and operating instructions for volunteers and participants; attend pertinent training and develop training for co-workers.

Licenses/Certification Required: Possess a valid California Class C Driver's license by date of appointment. State law mandates that all public employees having direct contact with minors must be fingerprinted as a condition of employment.

Environment/Working Conditions

 Environmental Conditions: Due to the nature of work assignments, the incumbent typically works in an office environment. However, some assignments may require the incumbent to visit outdoor or indoor field work sites to accomplish tasks.
 
Physical Conditions: Due to the nature of work assignments, the incumbent must have the ability to speak clearly; hear conversation in person, over the telephone; vision to read written and typed materials; have manual dexterity to operate a variety of office equipment and a computer keyboard; and lift and carry items weighing up to 15 pounds.
