We are seeking a dedicated and organized bi-lingual Care Coordinator to join our team. The role of this job is to contact members of the community who are among the disabled and high health risk to connect them to much needed resources. The ideal candidate will possess strong communication skills, both Spanish and English fluency, a basic understanding of medical terminology, strong computer skills (proprietary software is used, so no specific software skills necessary), excellent listening skills, the ability to empathize with others and to communicate that over the phone in a kind, caring way, and a commitment to providing exceptional team support We provide exceptional training and practice.

Duties

  • Serve as the primary point of contact for clients, addressing concerns and providing information about services that may be available to assist them
  • Schedule appointments for clients when necessary
  • Complete applications for clients when necessary
  • Maintain accurate client records and documentation in compliance with privacy regulations.
  • Assist with the coordination of client care, including follow-up communications and reminders.
  • Collaborate with medical or dental staff to ensure smooth operations and optimal client experience.
  • Handle administrative tasks such as data entry, and managing correspondence.
  • Utilize medical terminology effectively to communicate with clients and staff regarding care plans
  • Perform needs assessments over the phone

Qualifications

  • Previous experience in customer service is a must
  • Strong organizational skills with attention to detail to manage multiple tasks efficiently.
  • Excellent interpersonal skills to foster positive relationships with clients and team members.
  • Experience in providing administrative support in an office environment is beneficial.
