Position Title: Case Manager
Location: Vancouver, WA

Summary:
LHH Recruitment Solutions has partnered with a leading healthcare organization in Vancouver, WA, to seek dedicated Case Managers to join their team. This role involves assisting adults with disabilities and elderly individuals to assess their needs, authorize services, and ensure they remain independent in the community. Case Managers work to prevent placement in nursing homes or other institutional settings, facilitate timely returns home after short-term hospital stays, and provide ongoing support for those with functional impairments. The ultimate goal is to develop a service plan that maximizes the client’s independence while addressing immediate needs.

Key Responsibilities:

  • Conduct comprehensive assessments and reassessments to evaluate client needs.

  • Develop personalized care plans focused on client outcomes.

  • Implement short and long-term service plans, authorizing in-home and community services.

  • Resolve client concerns regarding service quality and delivery.

  • Coordinate with service providers and community resources.

  • Plan for case termination and follow-up.

  • Collaborate with in-house nursing staff for case consultation and service planning.

  • Maintain accurate client records (manual and digital).

  • Work closely with community providers, including mental health and disability organizations.

Knowledge, Skills, and Abilities:

  • Familiarity with local resources and services for the elderly and disabled.

  • Experience in direct functional assessments, service planning, and implementation.

  • Ability to work in diverse, multi-cultural environments, including using interpreters.

  • Strong organizational skills for maintaining both paper and digital records.

  • Excellent communication and relationship-building abilities with clients, caregivers, and service providers.

  • Capable of working independently, especially in the field, while managing deadlines.

  • Proficiency in English (verbal and written) and capable of using a computer for extended periods.

Minimum Qualifications:

  • Master’s Degree in behavioral or health sciences with at least one year of social service experience OR

  • Bachelor’s Degree in behavioral or health sciences with at least two years of social service experience OR

  • Bachelor’s Degree with four years of social service experience.

  • Valid driver’s license, reliable transportation, and minimum state-required vehicle insurance.

Working Conditions:

  • Travel to client homes and various community settings, including non-ADA accessible locations, is required.

  • After a trial period, employees may work remotely up to two days a week.

  • Must be able to perform duties that include standing, walking, sitting, and occasional lifting (up to 20 lbs).

Pay Details: $60,000.00 per year
