Supportive Housing Case Manager
**Job Summary**
Compass Career Solutions is seeking a highly skilled and compassionate Supportive Housing Case Manager to join our team. As a Supportive Housing Case Manager, you will play a critical role in providing supportive services to clients in need of housing assistance.
**Responsibilities**
* Conduct thorough assessments and interviews with clients to determine eligibility for supportive services
* Develop and update comprehensive action plans, case notes, and other documents related to a client's file
* Provide ongoing life skills development to assist clients in improving independence and quality of life
* Connect clients to medical, mental health, and other supportive counseling services
* Establish a network of community services and conduct appropriate referrals to clients
* Guide clients through the process of application, qualification, re-determination for SSI, SSDI, Medicaid, Medicare, SNAP, and other services as needed
* Advocate daily on behalf of clients
* Conduct visits in the client's home as needed
* Enlist the support of property management as appropriate for wellness checks
* Track and report outcomes, providing data updates as directed
* Attend and actively participate in scheduled internal and external meetings
* Travel throughout the area and provide transportation to clients as needed
* Represent Compass Career Solutions at professional meetings, community events, and to members of the community
**Requirements**
* Ability to use assigned technology as directed in the community, including laptop, cell phone, and designated software
